Below is a list of Undergraduate forms available in the Curriculog Proposal System. To complete a proposal, access the Curriculog website and select "New Proposal".

Course Forms

Course Change Form

Used to propose changes to an existing undergraduate course. The Short Form is for changes to: Course Number, Course Description, Course Requisites, Grading Mode, Schedule Type, and Restrictions only.

Step by Step Completion

Step 1: Start Curriculog Submission (Draft) and Print a Copy In Curriculog, select: Undergraduate Course Change Form

Step 2: Complete Course Concurrency Form (If Applicable) Complete this form if the proposed course changes may create overlap with an existing USF course.

Step 3: Use Template to Create Syllabus/Syllabi

Step 4: Complete EDU Cover Page

Step 5: Submit Cover Page, Syllabi and Curriculog Draft in DocuSign for Signatures

Step 6: In Curriculog, Attach Signed Cover Page, Syllabi and Submit

Step 2: Complete Course Concurrency Form (If Applicable) Complete this form if the proposed new course may overlap with an existing USF course.

Step 3: Use Template to Create Syllabus/Syllabi

Step 4: Complete EDU Cover Page

Step 5: Submit Cover Page, Syllabi and Curriculog Draft in DocuSign for Signatures

Used to either: (1) Discontinue an existing undergraduate course at USF, or (2) Remove one or more attributes from an existing undergraduate course at USF.
